The highlight for me at Forma is the pasta dalla forma, which is pasta tossed in these gorgeous wheels of cheese. The tortellini in particular is divine. However, the menu has lots of highlights, including small plates and mains. Great wine list. Cons are that service can be a bit hit and miss. Like most restaurants these days, the noise level is off the charts. Last time in our server forgot two of our small plates, but frankly I think he just couldn't hear us when we were ordering. I am not sure why restaurants are all so noisy these days, but it's a trend I hope ends soon. The seating is also a quite snug. You could be very close to another dining party. Again, that seems to be more and more typical these days, especially with the horrendous emergence of the "community table," which luckily Forma has not adopted. Forma is so popular that it could certainly fill a larger space elsewhere and give their patrons some room. These cons of the experience are mainly commenting on current trends that Forma has unfortunately adopted, but the food, cheese selection, and wine list are so good I would suffer through most anything to have a meal there. Make sure you get your reservations ahead of time as it fills up fast!

Although this lunch meeting was the third time for me, it was only the first time at Forma Restaurant for my guest. She was as pleased as I always am. The menu continues to be interesting and the presentation of food is as creative as ever. We were able to chat with each other without having to try to filter out noise. This is a huge plus considering the high noise levels at so many other restaurants. And. . .it's in my home town, Santa Monica, just a hop, skip and a jump from my home. It's a go to restaurant, although I never have had the pleasure of dinner there.
This is a thoroughly outstanding restaurant in Santa Monica away from the beach (easier parking!) The menu emphasizes the creative use of fine cheeses and pasta, but includes a large variety of other well-conceived and superbly-executed dishes based on meat, fish, salad, or vegetarian/vegan themes. Many gluten-free options, clearly marked, add to the available riches. I could happily eat here every night for a week, ordering something different each time. It's not cheap; tables are closely spaced; it's somewhat noisy; but all is forgiven for the high quality of food and service.
Always an amazing experience at Forma. Make sure you order one of the Dalla Forma - their specialties.
We have been to Forma many times and it is always consistant and delishes. The wait staff is friendly and attentive! We will be back.
Forma’s pasta tossed in fresh cheese is amazing. We sampled 2 cheeses and loved the fettuccine Bolognaise. This is a spot I will go back to many times!

In keeping with current restaurant trends, one has no tablecloth and somewhat spartan, though not unpleasant, decor. Maybe I'm old-fashioned, but when main courses enter the high twenties and thirty dollar range, I find that a bit disconcerting. The food is splendid, and there are enough options to cobble together a meal that is not too expensive or vegetarian (the cheese components make vegan more difficult) or non-mammalian. The service is attentive without being overwhelming or, worse, snotty. Though I know how to pronounce Chitarra Cacio e Pepe, not everyone does, and they won't sneer at you here if you foul it up.
I love the pasta dish and the wine selection is wonderful. The staff paired our chaucitere and cheese perfectly. We like to arrive on the early side since it can get so loud!
It's always amazing, we love Forma! We are never disappointed. Always look forward to the next time
Simply classic pasta. Not a fan of paying $4 extra for bread at the table.

I was in the area for work and have been wanting to eat here for quite awhile. My lunch was perfect... from the amazing citrus avocado salad to the cheese wheel pasta. I will definitely be back!

Very good Santa Monica modern style Italian on Montana. A bit too much oil on the food for my tastes but very good meal
The food was good, but the service was inexistent! Our waiter threw the menus on the table, did the same with the dishes we ordered, never came back to ask if we needed refills on our waters and iced teas...did not take the plates off the table once we finished...Pretty bad. We mentioned that this was a Birthday lunch...but not a shadow of a candle or a dessert to be seen...We still left a 15% tip, just to show appreciation for the people behind the scene, the cooks, bus boys etc...but we are not hurrying back to Forma...I
The short rib pasta was full of delicious and our dessert of 3 bleu cheeses with port pairings was a perfect finish to n amazing dinner!

Pastas are unreal. Highly recommend the cacio e pepe, arancini and gnocchi. This time we tried the branzino which was good but I'd say stick to pasta as it's their specialty. Service was fine and ambiance was really nice, especially on a rainy night.
We went on a Fri night. The food was amazing. Everything was delicious. The big challenge was deciding what to choose! Tables were very close and it was loud, but the food made all that worthwhile.

Amazingly good pasta. Friendly service without interrupting us. It does have a crowded feel and the noise level was quite high. It was difficult to have a conversation
For her 16th birthday our daughter wanted to try the Forma pasta in the cheese wheel. It was good but not enough for 2. She split it with her friend but they could have each had their own order. I had the BLT & it was FANTASTIC!! The salad that came with it had a very nice light dressing.
This was the fourth time we have eaten at Forma. It keeps getting better.
