We always enjoy going there, they have an amazing selection of local and international cheeses,,their wine list its very good, and the food, o the food, always great, their pastas, pizzas and entrees are just to die for, I love the shrimp and calamari with beans, and the cosse and peppe its just amazing .

So loud we couldn’t hear ourselves speak. Cheese selection was fantastic. Wines by the glass were disappointing and the food was average.
Forma is our favorite neighborhood spot. Food is delicious. Service is always good and often terrific. Pastas and cheeses are wonderful. My go-tos are Chicken sandwich (best you have ever had), Greek Chicken and Colorado Lamb. Make a reservation because Forma is busy 7 nights a week with a good crowd. Great spot for lunch too although the menu is the same as for dinner.
Excellent food, service, wine and atmosphere! The restaurant was full when we came even though it was late evening on Monday, so reservations are advised.
This place blew my mind! Already planned a trip back for next month. Some of the pastas are homemade and you can really tell. The cheese selection is outstanding. And portions are worth the money. Left full, with leftovers and didn’t feel guilty about what I spent.

The hostess on the phone was the only less than stellar part of this dining experience. The food is always excellent and the wait staff was also terrific. We ordered too much but it’s hard not to want it all!

Great culinary experience authentic Italian food
This is my third visit to Forma. Each time I am impressed by the meal. The pastas and fish are delicious. It's a real neighborhood gem. Always busy, always enjoyable.
This is an excellent Italian restaurant. The pasta dishes are superb. The roasted vegetable plate was delicious and generous. The bread was fabulous. Our waiter, Marco, was friendly and helpful.
The gelato and sorbet were the perfect way to end the meal.
We go to Forma whenever we are in Santa Monica and we will return!
If the dessert had been better I would have given 5 stars. The Italian doughnut with custard tasted doughy and it was disappointing. Everything else was superb! Maybe you can get a new pastry chef.
Great place to try.. if your a cheese lover and pasta lover then this is pure bliss.. get the chitarra pasta it's amazing... the wine list was great and the cheeses were just magnificent

I was happy to get the front table for the 8 of us. Service was a bit slow, but not terrible.
One of the best varieties of dishes for cheese lovers but also had enough options if you aren’t. Perfect service from hostess, waitress, server, people making pasta in cheese wheel. Limited wine selection for an Italian restaurant but what was available was of good quality.
Tortellini, octopus & cheese as individual selections are must gets!!! 2 misses were the cacio e pepe (dry with no flavor - too al dente - I’ve heard of magical plates of this dish from my friends that have visited Italy, so I wanted to get a taste of what they were talking about, but it was a huge disappointment - it tasted like boxed pasta with cheese sprinkled) & burrata (nothing special - I’ve had better at mid-range priced restaurants). It was really a hit or miss when it comes to picking dishes.
Would definitely be back when I want a wine, cheese & pasta dining experience.
I would give Forma 5 stars but it is really noisy... That's true of a lot of places, I know. The food is excellent and different. Go there for sure!

I’ve been to Forma before and was excited to share the experience with a good friend of mine last night. She had never been and loved the place.
Definitely get the mushroom tortellini, I believe it’s the best item on the menu
Showed up for an early dinner and the bar was popping. The dining room was empty but filled up quickly and was packed by the end of our meal. Great spot to try out different cheese and drink wine.

Forma Restaurant does not disappoint. It offers a full range of Northern Italian cuisine. On several visits my guests and I have sampled most of the menu. We have not ordered anything that we did not like.

Froma is one of the best Italian Restaurants on the Westside. Thy serve authentic homemade Italian pasta. Their signature dishes are the pastas tossed inside a giant cheese wheel. Its the ultimate creamy entree. Personally, it is best to share that because it is a lot of cheese to eat yourself! Share the pasta and order a soup, and/or a salad, or a grilled fish. The seating is tight, but so worth it!
Food is always great there, great ambiance and what I love most is the huge selection of cheese varieties they have! That puts them over the top for me!
We were there early Sunday so it might be noisier later when full but the food was outstanding and the service was impeccable. I originally went solo at happy hour and was happy to sit at the bar and eat delicious food and watch a game. The bartender made me feel right at home. So made reservations (you need a reservation!) to go with friends so we could order family style and try a number of dishes. We were not disappointed by any dish we ordered. I plan on going back frequently with my boyfriend and to keep cost down order a starter or two and a main. So many cheeses to try too! The friends I went with are foodies and they raved over every dish.
We had been meaning to go to Forma for a while and it was well worth the wait. Prices were very fair and the food was amazing!
Lets see....i was super excited about coming here, but it didn't meet my expectation. First of all on their site it says do not request for booth seating but i did anyway because I chose this place to celebrate. My partner and i chose this spot to celebrate many things, anniversary, promotions, everything, and upon getting there, there booth seats open but they sat us in a teeny tiny table cramped in the middle of other people....so um why couldn't they just honor my request if the seats were available?? That was the first disappointment. This place is way smaller than it looks online. If you been to Barton G, Be prepared to eat that close to your neighboring guest as well. You can't even have a conversation without hearing the people next to you talk. I mean literally you can hear every single word they say..it was quite annoying having dinner and also hearing a annoying women complain about her life to her friend. You can't even enjoy your own conversation because everyone is so close to you. So hard to enjoy my dinner with so many awkwardness all night. Our waitress and everyone was friendly so i guess thats the only good thing? Also if you wanted a picture of the pasta wheel, you have to walk through all these people sitting right in front of it. Awkward?? I ordered something to be made from the pasta wheel, and they don't even tell you when its yours being made so you can get a picture, even though i told the waitress i want to take a picture of my pasta being tossed and she told me she'll let me know but she never did. No cocktails, just wine and beer only. Over priced for mediocre food. This will be my first and last here. Okay bye!
We have been to Forma a number of times and, while noisy with crowded tables, we always enjoyed the food in the past. Not so this last time. The menu has changed and the individual dishes that we had were both strange and not well thought out. Specifically:
Chicken Meat Balls were oddly prepared with no flavor at all and a watery base
Greek Chicken was just ok. Would have been a acceptable lunch dish but clearly not adequate for dinner
Sole which was always good was just fair and, frankly, boring
Octopus was totally unexciting
You can see that this has become a place that I would not go back to and would not recommend
