The ambiance in this small restaurant is a joy. We feel transported to Italy in the heart of North Beach. The food is tasty, old world Italian and the desserts are outstanding! The owners greeted us like old friends at only our second visit. We will be back! Caio!

We have been going the Cafe Macaroni for over 20 years. It’s a tiny funky place, and we love it. The food is very good. We started with a glass of Prosecco and the burrata appetizer. The cheese was soft and fluffy, yummy!
We always get the squid ink pasta with seafood. The clams were particularly delicious. Our friend ordered the spaghetti and meatballs which was really good. The lemon cake dessert was also delicious. The waiter is chatty with a thick Italian accent which just helps the feeling of having been transported to a fun and special place. It always feels like a celebration

Caffe Macaroni is a charming little Italian restaurant on the edge of North Beach, which is San Francisco's Little Italy. Chef/owner Mario Ascione is a warm, charming, down to earth host who welcomes his customers like they were guests in his home. His food is the real deal, like food you find in southern Italy, near Naples, which is his home. While you're free to choose what you like from the menu we put ourselves in Mario's hands, asking what he recommended. He brought us a delicious array of antipasti, a luscious bottle of house Sangiovese, amazing seafood pasta and a few deserts, all of which we shared. The evening ended with complimentary glasses of Orangecello. Our meal was delizioso and we eagerly await our next amazing dinner at Caffe Macaroni.
Excellent as always. Mario is the consummate host. The menu is vast for such a small kitchen and restaurant. Everything is authentic and delicious, no matter what you order!

Dinner is always good at Caffe Macaroni. Owners are attentive as are the rest of the staff. I wanted to take an out-of-town guest to an authentic Italian restaurant in North Beach and we were not disappointed. The Chicken with Gorgonzola sauce was super and my guest enjoyed her mussels and pasta. Of course, we had to finish off the meal with tiramisu - delicious!

Overall great but we did not appreciate the waiter telling us we could not have parmesan cheese over our linguine with clams... I appreciate them being particular about the end result of their dishes but the customer is always right.

Would definitely recommend Caffe Macaroni. I took my wife and it is definitely a quaint restaurant. It is a little small, but that makes for an intimate night. Service was great and food was perfect.
Ravioli was served cold. Sent it back and server returned it just slightly warm, still not hot. We were never asked if it was okay. Soup of the day was described, we ordered it, and it came as a completely different soup. Server just said sorry. Appetizers arrived at the same time as the main dishes. Very small portions for the price, especially the pesto pasta and salads. Their internet was down so we couldn't add the tip onto the credit card for some reason. When we gave all of this feedback to the Maitre D, he became defensive and blamed our entire experience on the fact that their computers weren't working. We have been to this restaurant several times with a great experience, but we will not be returning.
Good meal. Our waiter, Claudio, made us right at home. We discussed everything Italy: coffee, travel, and food! Enjoyed a bite on the second floor in front of the BIG picture window with shared seating!

A memorable authentic Italian experience. My son and I were visiting San Fran for a few days and settled on this little gem after reading dozens of reviews for authentic old school Italian and we were not disappointed! Caffe Maccaroni is a tiny little place with room for 11 inside and four small tables outside. We started with Caprese and the best part was the amazing olive oil, very fresh and simple. I had a halibut special over al dente pasta with a light but very flavorful tomato sauce, all cooked to perfection. My son had a cheese and spinach ravioli and loved it. We closed with lemon cake very delicate, light and delicious. The staff here were all very good, all Italian, highly recommend!
We never miss going to this little joint whenever we visit SF. Seriously the best.
