Caffe Macaroni is a delightful restaurant. Cozy in all the best ways. They know their food and cook it just like you were in a grandma's kitchen.

Owners Mario and stefania are the real thing. They provide authentic Napolitano cuisine. Even the menu is written in Napoletano dialect.

Wonderful memories of everything about Caffe Macaroni from years of being able to just drop in after work or at lunch. Now it's a destination choice for us but just as charming and delicious as previously. Mario and the fellows that back him up are just great.
Recently there was an article circulating about the sad state of affairs for restaurants in SF -- just based upon how busy Caffe Macaroni was from 5:30p to 7p -- clearly they've risen above the fray!

We had a garlic bread dish, seafood (Linguini di mar), boar (Rugu di cinchiale con pappardelle) paired with a great wine (Scarpeli) and Peroni.
Attentive service. A local gem.
If you want a quieter experience, ask to sit upstairs. If you want to be in the heart of things, ask for upstairs or the sidewalk.
We love Caffe Macaroni. It is best to make reservations as it is such a small little place. I would suggest that you not waste your money on the glasses of wine (go for a bottle or no wine at all) as it is really poor quality. Perhaps the worst wine I've ever had. But the food is absolutely delicious!

The proprietor (Mario) knows what he is doing. Didn't need a menu. He suggested the perfect pairing for the Barbara we were enjoying. Please support this AWESOME bit of S.F. culture!
Mario the chef's Neapolitan food is spectacular: many Napolitan' specials like shellfish risotto, baby octopus pasta, and various fresh -- very fresh -- fish combinations, several with pasta and Calabrian chilli pepper. Also, he somehow gets calamari so fresh that it melts in your mouth served as an antipasto with white beans, lightly sauteed (not deep fried!). And some of the dessert cakes are made by patronne Stephanie, are fabulous. I sound like an advertisement, but I'm just a regular local patron who thinks Caffe Macaroni is a treasure.
We come here every time we come to SF. The food is fantastic and the wait staff superb.

Extremely small place. We ate in room upstairs with low ceilings. Very unique. Food and service superb. Venison ragu pasta excellent. Entrees came out piping hot.

Excellent food and service. The chef was extremely friendly and made rounds of the tables and gave us shots of Limoncello on the house just because, no reason. Everything was great. A little loud for a first date.

Absolutely on of the best Italian meals I have ever eaten!! I’ll be dreaming about the Panna Cotta for days 😂 The waitstaff and owner were very attentive and pleasant...which made the experience a great one as well! We will be back!
This whole-in-the-wall continues to give an authentic Italian experience that makes you long for another trip to Italy. For such a small restaurant, the menu is very broad and the food is excellent. Fresh made pastas are a highlight.
I came for an item that was specifically on the menu; A sardine pasta dish. It is a dish that my Italian grandmother often cooked. They did not have it. The owner said that he would substitute another fish for the sardines but that the rest of the dish would be the same. The food was delicious. However, in addition to the portion being quite small, he charged me $11 more than what was printed on the menu! Not acceptable. (Also, the bread was as basic as it comes. Like something one would buy from Walmart...)

I love Caffe Macaroni. It would be easy to assume it's just a tourist trap, but do not be fooled. This is authentic Italian food made by knowledgeable chefs and served by true Italians. We come here almost every year for my boyfriend's (an admitted Foodie) birthday. Our mantra is always to never order from the main menu. We exclusively order the "specials." This year for the first time, the specials were actually on a menu, rather than told to us by our waiter. We started with a delicious burrata and caesar salad, followed by two pastas - one with squid, the other with venison. Both were amazing, mostly because of the rich and balanced sauces. Our main was an antelope steak and it was tender, juicy and perfectly cooked and seasoned. The only disappointment this particular evening was the very loud party near us that were doing a lot more talking than eating, and having some sort of altercation. But otherwise, as every year, Macaroni did not disappoint!
We sat on the second floor and the space was kind of cozy. The service was good and our appetizer and dinner out in a reasonable time. My husband loved his black ink pasta dinner. Overall, we enjoyed our dinner there.
Have had dinner here about half a dozen times and the food and service is always great. It’s what an Italian restaurant should be. When my mom, who is from Italy, says “this is the only restaurant in SF I want you to take me to when I’m in town” you know it’s good.

Caffe Macaroni is delightful, a tiny gem in a box of the semi-precious. The food is excellent, the owner and staff are charming and helpful, and the wine list is truly interesting. We let them tell us what to order, and everything was superb. Tutto bene cui.

The food is superb and reminiscent of my tasting heaven in Italy many years ago. Mario and Stephanie, the owners, and their entire staff are warm and welcoming. I have been eating at Caffe Macaroni for years and have recommended the cuisine to friends in Australia, England, France, Germany and around the world! Unfortunately, the restaurant is very small, but eating outside on Columbus Avenue is like being in Italy! San Francisco, open your Golden Gate and keep it open to all.
Greasy and flavorless. Stale Bread. Cramped and loud.
Greeted immediately and given the option to sit outside or in. Drinks came quickly, walked thru the menu and appetizer and entrees ordered easily and we were excited to try them. Both the appetizer and the entrees were well-timed to the table and everything was terrific. Wonderful kind prompt service in a great location with part of the charm being how small and intimate the space is. We liked all we had and the service, likely to return.
Feels like being in Italy. We loved the food, the recommendations from Mario were fantastic. This is a small cozy place with absolutely great food. We enjoyed it immensely.
A San Francisco institution. This tiny little restaurant has been around for quite some time, and over that period, the quality has never slipped. The owner walks the dining room, engaging with everyone. The upstairs dining room, with it's low ceiling and huge window to the street, is the best place to sit, eat and watch SF go by.
The sauces are unreal. Simple, fresh and flavorful, you cannot go wrong with any selection. Wine by the glass is an excellent value, with a generous pour and full choice of varietals. Prices are fair and in line with other establishments in the area. Service was friendly and fast. We enjoyed a very good meal and will certainly return again.
